I am on a low glycemic diet and have eliminated all meat and chicken.  I only eat fish on a rare occasion.  My diet mainly consists of green salads with a little tomato, almonds, tofu, black beans, fat free cottage cheese, fresh or frozen vegetables, 1 banana in the morning, and boiled eggs.  I had been eating an apple and an orange a day along with 1 piece of low glycemic whole wheat bread.  I was diagnosed with type 2 diabetes in February of this year and nearly lost my life.  I am currently on 2000mg of Glumetza per day.  My endocrinologist seems unaffected by the fact that my sugars run from the low 100's to about 280 during the course of each day.   He says not to worry, so I have made an appointment for a new endo.  I feel alone in this fight at the moment and very frustrated.  I do everything right except for the inability to exercise due to severe long standing spinal injuries.  Ambulating is difficult...exercise out of the question.  I walk as tolerated...very slowly and with much pain. 
Now that you know my beckground, I would like to know why, whenever I eat just one regular sized orange  or a small mac apple, does my blood sugar hit almost 300, a rise of about 100 to 200 points in minutes and lasting for 2 hours.  With the increase, I usually feel sick.  I just don't understand.  I have done so much research and feel that my eating habit are the best they can be.  Any advice?
